The Diagnoses module allows you to record details of a patient's past and current conditions relevant to their care. This includes the onset and resolution dates, and notes regarding the issue.
You can add a diagnosis by selecting the plus icon in the Diagnoses module and filling out the details of the window.
- Click the + icon within the Diagnoses module.
- The New Diagnosis window will open.
- Enter the patient's diagnosis.
- Enter the onset and resolution dates, if known/applicable.
- Tick the Active box if the condition if ongoing.
- Enter any notes as needed.
- Press SAVE.
Note: If there are no matches when adding diagnosis name, you can simply move to the next field and your entered text will be retained.
The new diagnosis will be added to the patient's Diagnoses module, along with the Onset date (if entered). If the diagnosis is no longer active, it will appear in gray text.
If there is a change in a patient's diagnosis, such as a resolution to the condition, you can click on the diagnosis listed in the Diagnoses module to enter an update.
In the Edit Diagnosis window, you can update the Onset or Resolved dates, tick or untick the Active box, and enter additional notes about the patient's condition.
If the diagnosis was entered mistakenly, you can also select the DELETE button to remove the diagnosis from the patient's record.
If a patient has many diagnoses, you may wish to hide those that are no longer active. This can be done by selecting the menu icon on the Diagnoses module to toggle between viewing Active Only, Inactive Only, or Both.
Any diagnosis that isn't ticked as Active will be display in a gray text, allowing you to differentiate between those that are active and inactive when the option to view both has been selected.

The Diagnoses list are provided by SNOMED and cannot be added to or edited.
You can add your own Diagnosis name ad hoc, however it will not be saved to the list for future use.
